EPIC analysis (nearly 267,000 participants) found that among UPF subgroups, associations with cancer and cardiometabolic multimorbidity were "most notable for animal-based products (HR: 1.09, 95% CI: 1.05, 1.12), and artificially and sugar-sweetened beverages (HR: 1.09, 95% CI: 1.06, 1.12)," while "other subgroups such as ultra-processed breads and cereals (HR: 0.97, 95% CI: 0.94, 1.00) or plant-based alternatives (HR: 0.97, 95% CI: 0.91, 1.02) were not associated with risk." The authors themselves wrote that the findings suggest "more nuanced subgroup analyses of ultra-processed foods" are warranted. Lead author Reynalda Córdova stated the results emphasise "the importance of a differentiated analysis of ultra-processed food subgroups."
This subgroup heterogeneity is echoed elsewhere. The Fang et al. BMJ 2024 cohort study of US health professionals concluded that higher UPF intake was associated with "slightly higher all cause mortality, driven by causes other than cancer and cardiovascular diseases," with subgroup-specific hazard ratios reported separately rather than a uniform effect across all UPFs.
