The claim that fasted exercise causes greater fat loss than fed exercise is misleading. It is true that low-to-moderate intensity aerobic exercise performed on an empty stomach burns slightly more fat as fuel during the session itself. However, controlled trials and systematic reviews, including a randomized study by Schoenfeld and a 2017 meta-analysis, find no significant difference in actual body fat loss between fasted and fed exercise when total calorie intake is the same. Body fat change is driven by overall energy balance across days and weeks, not by which fuel source your body uses during a single workout. The higher in-session fat burning is offset by changes in fuel use later in the day. Fasted exercise is not harmful for most healthy people, but it is not a shortcut to more fat loss.

The claim conflates two different things: fat oxidized during a single exercise session, and total body fat lost over time.
On acute fat oxidation during exercise: Aerobic exercise of low-to-moderate intensity performed in the fasted state induced a higher fat oxidation compared with a fed state. The Vieira et al. meta-analysis found a significant increase in fat oxidation during exercise performed in the fasted, compared with fed, state (approximately 3 g more per session) .
On actual body fat loss over time: The Schoenfeld et al. RCT randomized 20 young women adhering to a hypocaloric diet to fasted versus fed aerobic exercise for four weeks. Fasted and fed aerobic exercise produced similar fat loss when combined with a hypocaloric diet in young women over 4 weeks. Fat mass decreased significantly in both groups (P=0.02 for time) but no significant between-group difference (P=0.88).
The Hackett & Hagstrom systematic review and meta-analysis concluded: The data shows minimal changes in body mass and composition following aerobic exercise interventions in both fasted and fed states. Furthermore, performing exercise in a fasted state did not influence weight loss or changes in lean and fat mass.
A more recent expert summary reiterates: a 2017 systematic review demonstrated that a fasted exercise training program doesn't seem to translate into long-term differences in body fat loss. The reason is that aerobic exercise in a fasted state causes you to burn more fat as a fuel when measured at a single point in time, so it wasn't a big leap to assume this would translate to longer-term fat loss , but this assumption does not hold up. As one summary puts it, short-term transient spikes in fat oxidation are meaningless in isolation. It's the long-term balance over days and weeks that dictates whether you lose or gain body fat. A small spike in fat burning during fasted exercise is offset by shifts in substrate use over the rest of the day.
